How to get an NPI number?

According to the Health Insurance Portability and Accountability Act, an individual or organization conducting billing transactions for medical services rendered must obtain a National Provider Identifier (NPI) number. The NPI helps keep medical billers accountable and also facilitates the billing transaction process. There is no cost to obtain an NPI number. You can apply in one of three ways: online, by mail, or by phone.


Apply online

1 Visit the official website for the National Plan and Provider Enumeration System. A link to this site is in the “Resources” section.

2 Click the link “Online Application For an NPI.”

3 Complete the NPI form online by entering all the requested information. For example, you will need to enter your social security number, the name and address of your practice, contact person, provider type, taxonomy code, and license information. Click “Submit” when done.

4 Make a note of the tracking number for your NPI application. The tracking number is automatically generated when the form has been submitted successfully.

5 Wait to receive your NPI number. This can take up to 15 business days. If you do not receive your NPI number within 15 business days, you can telephone the NPI enumerator at 800-465-3203.

Apply via mail or phone

6 Download and print a copy of form CMS-10114 from the Medicare website. A link to this form is in the “Resources” section.

7 Complete Form CMS-10114 by entering all the requested information.

8 Send the completed form to the following address:
